NO SHARING OF MOBILE DATA

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors that provide operational support services (such as our patient messaging platform and customer service) is permitted strictly to facilitate our practice communications. Message frequency varies depending on your interactions with our practice, upcoming appointments, and ongoing care schedule.

4. FEES & RATES

Lake Vision PLLC does not charge a fee for text notifications. However, standard message and data rates may apply according to your wireless carrier agreement.

5. OPT-OUT INSTRUCTIONS

You can cancel the SMS service at any time. Text "STOP" to any text message you receive from us to opt out. After sending "STOP", you may receive one final confirmation text acknowledging your opt-out.

7. CARE NOT CONTINGENT ON CONSENT

Wireless carriers are not liable for delayed or undelivered text messages.
